THE BASKETBALL EDGE CAMPER PRIVACY POLICY

 

In compliance with Alberta legislation to protect the privacy of Albertans and because The Basketball Edge is committed to the highest standards, we have developed the following privacy policy.



Why do we collect your personal information?

  • to communicate with you about our camps and services
  • to ensure the safety and health of all athletes
  • to provide insurance for all participants and coaches
  • to manage our business and operations
  • to develop, enhance, market or provide services



What Information is collected?

  • Athlete’s name, date of birth and gender
  • Athlete’s current school
  • Athlete’s Health Care Number
  • Athlete’s medical history
  • Parent/Guardian name
  • Postal Address
  • Email Address
  • Phone number
  • Emergency contact information



Who is the Information shared with?

Basketball Alberta: The Basketball Edge will automatically purchase a membership with Basketball Alberta for each athlete participating in any Edge camp. The athlete’s name, gender, date of birth, postal address and phone number is shared with Basketball Alberta to provide insurance coverage for each athlete.

Health Care Providers: Should an athlete require emergency health care while participating in a camp, any necessary personal contact information and health information will be shared with the health care professionals to ensure adequate care is provided and that the parent/guardian can be contacted accordingly.

Third Party: The Basketball Edge may confirm an athlete’s registration in a particular camp, however any other personal information (address, phone number, etc) will not be disclosed without your consent.



How is your personal information protected?

  • Only staff and owners of The Basketball Edge have access to your information.
  • The Basketball Edge will not collect, use or disclose your personal information for any reason other than those stated above.
  • The Basketball Edge will retain your personal information only for the period of eligibility of the athlete for various Edge camps and services.
  • The Basketball Edge will strive to keep your personal information as accurate and up to date as is necessary for the purposes identified above. We ask that you advise us of any changes in your personal information that we need for the purposes identified above.
  • The Basketball Edge will only release your personal information to a third party once signed consent has been provided by you.



Consent

By registering for any camp with The Basketball Edge you consent to the collection, use and disclosure of your personal information as described above.

 

THE BASKETBALL EDGE REFUND POLICY



            Refund requested prior to Registration Deadline will be granted less a $25 administration fee.

           

No refunds will be granted after the Registration Deadline.

 

THE BASKETBALL EDGE PARTICIPATION AND WAIVER POLICY



Both participant and parent/guardian agree:

To be solely responsible for any personal property loss or damage or any injury sustained, unless such loss damage or injury was the result of sole negligence of The Basketball Edge.

That there are inherent physical risks associated with basketball activities and understand and accept these risks.

That the participant is physically, mentally and medically able to participate in all the activities of this camp.  If the camper has special needs, The Basketball Edge cannot provide one on one supervision or medical support.  Participants who are in regular school classrooms and have an aide are normally able to attend camp. They must be accompanied by an aide provided by the parent at camp.

That should the participant be injured during the camp I/we give permission for The Basketball Edge staff to provide sports first aid and obtain medical treatment in an emergency situation.

To grant permission to The Basketball Edge to use the participants name, statements, photograph, game performance or likeness in any live or recorded form, in whole or in part, for promotional and commercial purposes or otherwise, without any additional consideration.

The Basketball Edge reserves the right to request any participant to withdraw from the camp if the participant is not acting in an appropriate and responsible manner; and to cancel any camp without a refund for the remaining days.

The Basketball Edge reserves the right to change the camp location from one gymnasium to another gymnasium, within the same community, without any additional consideration.

The Basketball Edge reserves the right to make a coaching change without any other additional consideration.

I hereby waive for myself, my heirs, my executors, and my administrators any and all rights for damages that I may have against any individual, or organizer, or staff of The Basketball Edge’s programs, their agents, representatives, successors, and assigns, for any and all injuries that I may have sustained during the course of the above programs.
